You'd be surprised how many kids actually enjoy being held like that. Two out of three of my kids liked being held upside down. The other child would scream if she was upside down at all. You can tell by the giggles. One of them actually liked being swung a bit just like the kid in the photo. That child is currently my little thrill seeker.
I could pick my kids up by one leg and swing them onto my shoulders. Then they'd grab my curly hair and laugh and giggle. A little rough play serves to strengthen them up.
Further, you can get a kid into a tight-necked shirt this way too. One parent holds the child upside down while the other puts on the shirt.
